The systematic review highlights that exposure to cold, whether through cold air or cold water, often leads to cognitive impairment, even in healthy individuals prior to hypothermia. Studies consistently demonstrate that a single instance of cold exposure can affect attention, processing speed, memory, and executive function. These effects may vary based on individual physiological responses to cold and the duration and intensity of exposure. Additionally, there appears to be gender differences in cognitive performance during cold exposure. While repeated exposure and potential acclimation may influence cognitive performance, more research is necessary to validate these initial findings.

The study observed the release of norepinephrine, a stress hormone, in six healthy men exposed to cold water immersion. Norepinephrine levels rose significantly within 2 minutes of immersion, reaching a peak after 45 minutes. Metabolic rate also increased substantially during immersion. Upon rewarming in warm water, norepinephrine levels peaked again before rapidly returning to normal, despite continued low core body temperature. This suggests that the body's response to cold, mediated by the sympathetic nervous system, can be rapidly activated or suppressed, depending on skin temperature.
