Does CPU Fan position matter?

As far as Air Cooling goes, CPU Fans are traditionally mounted at the front. However, due to a number of reasons, you might find yourself in a position where you are forced to have it at the back of the heatsink instead.

Does it really matter? What's the difference and how much of an impact does it have, if any?
